Every document below maps to a named Indian regulator. This is the same checklist our reviewers work from, published so there are no surprises.

Who this checklist is for

A clinical establishment seeking empanelment for medical value travel. 8 documents are mandatory and 3 are optional but speed up review.

Mandatory documents for hospital or clinic registration

1. Clinical establishment registration

Registration under the Clinical Establishments Act, or the state equivalent. In Maharashtra this is the Bombay Nursing Home Registration Act certificate issued by your local body — the Municipal Corporation, not a central authority. This trips up most first submissions.

2. NABH or JCI accreditation certificate

Mandatory for medical value travel and for Ministry of Tourism empanelment. Upload the full certificate showing the accreditation number and valid-to date. We verify the number against the accrediting body and publish it on your profile so patients can check it themselves.

3. Hospital PAN card

The PAN of the legal entity operating the hospital — the company, trust or partnership — not a director's personal PAN. We store only the last four characters.

4. GSTIN registration certificate

Your GST registration certificate. The legal name on it must match the PAN and the incorporation document exactly — a mismatch is the most common reason a submission comes back.

5. Certificate of incorporation, trust deed or partnership deed

Whichever applies to your entity — Certificate of Incorporation for a company, the registered trust deed for a trust, or the partnership deed. This establishes who legally operates the hospital.

6. Bio-medical waste authorisation

Your Bio-Medical Waste Management authorisation from the State Pollution Control Board. In Maharashtra that is the MPCB. Include the validity dates.

7. Fire safety NOC

A valid fire safety no-objection certificate from the fire authority. Must be current — an expired NOC suspends your listing automatically.

8. Board resolution or letter of authority

Signed by the directors or trustees, authorising a named person to bind the hospital to this portal's terms. Attach that person's identity proof in the same file. Without this, nobody at your organisation can legally agree to anything here.

Optional documents

State Pollution Control Board consent

Consent to operate, where your category of establishment requires one separately from the bio-medical waste authorisation.

ABDM Health Facility Registry ID

Optional but recommended. Your HFR registration certificate or unique facility ID from the Ayushman Bharat Digital Mission.

Cancelled cheque or bank letter

Required only if you will receive payments through the portal. The account name must exactly match the registered legal name. We confirm it with a penny-drop before any payout.

How we handle your documents

Files are encrypted at rest and every time one is opened we record who opened it and when — including our own administrators. Access links expire after five minutes. We keep each document only for the retention period set against it, then destroy it automatically while keeping the record that it was handled properly.

Why we ask for a masked Aadhaar rather than a full one

Where Aadhaar is offered as identity proof, mask the first eight digits — DigiLocker will generate a masked copy for you. We store the document image and never the number. A passport or a DigiLocker-issued document is better still, and any of them is acceptable.
