Generated illustration
A photograph of a real person beside a named doctor asserts that this IS that doctor. An illustration asserts nothing. It also removes any model release requirement and cannot be mistaken for a clinical claim. Replaced with a real portrait only when the doctor supplies one with written consent.

A generic ward photo beside a named hospital implies it is that hospital's ward. Stays illustrative until the hospital supplies its own photography.
